The doctor himself seems good.

The room is not equipped: orthopaedic doctors usually have mats, but here there was just a bare floor. There was nowhere to put the child so that he could walk around and the doctor could examine him properly. He just examined him on the couch. Children's rooms are usually bright, with toys, but here it was empty.
